Latar Belakang: Masalah Kesehatan Dengan Gangguan Waham Adalah Keyakinan Yang Tidak
Sesuai Realita Yang Di Pertahankan Walaupun Tidak Diyakini Oleh Orang Lain Dan
Bertentangan Dengan Realita Sosial.
Tujuan: Analisa Asuhan Keperawatan Melalui Intervensi Terapi Orientasi Realitas Pada Klien
Ny.M Dan Ny.S Dengan Masalah Keperawatan Waham Di Panti Sosial Bina Laras Harapan
Sentosa II.
Implementasi: Tindakan Keperawatan Yang Dilakukan Selama 4 Hari. Intervensi Utama Yang
Diberikan Kepda Ny.M Dan Ny.S Dengan Masalah Keperawatan Utama Wagam Yaitu Dengan
Melakukan Terapi Orientasi Realita Dengan SP.
Hasil: Evaluasi Keperawatan Pada Klien Ny.M Dan Ny.S Dengan Masalah Keperawatan Utama
Waham Catatan Perkembangan Setelah Diberikan Tindakan Melakukan Terapi Orientasi Realita
Dengan SP Selama 4 Hari Pertemuan Pada Klien Didaptkan Hasil Bahwa Waham Dapat
Terkontrol Dengan Pola Pikir Yang Berubah Menjadi Realita.
Simpulan Dan Saran: Melakukan Terapi Orientasi Realita Dengan Rutin Dapat Membantu
Merubah Pola Pikir Sesuai Realita.

Background: Health problems with delusional disorder are beliefs that are not in line with reality
that are maintained even though they are not believed by others and are contrary to social reality.
Objective: Analysis of Nursing Care Through Reality Orientation Therapy Interventions for
Clients Mrs.M and Mrs.S with Nursing Problems of Waham at Harapan Sentosa II Bina Laras
Social Home.
Implementation: Nursing Actions Performed for 4 Days. The main intervention given to Mrs.M
and Mrs.S with the main nursing problem of Wagam is by conducting reality orientation therapy
with SP.
Results: Nursing Evaluation of Clients Mrs.M and Mrs.S with the Main Nursing Problem of
Waham Developmental Notes After Being Given the Action of Performing Reality Orientation
Therapy with SP for 4 Days of Meetings on Clients The results were obtained that the Waham
could be controlled with a mindset that changed to reality.
Conclusions and Suggestions: Doing Reality Orientation Therapy Routinely Can Help Change
Mindset According to Reality.
